Who should not have a penoplasty? Men with body dysmorphic disorder or unrealistic expectations, patients with an untreated psychiatric condition, active smokers who cannot stop before fat transfer, patients with a bleeding disorder or an active genital infection. Erectile dysfunction is not a contraindication but must be treated first. Penoplasty covers the techniques used to improve the dimensions of the penis, in length or in circumference, to answer the aesthetic and psychological concerns of patients. Girth enhancement increases the circumference of the penis both at rest and during erection. It is the most frequently requested procedure. Surgically, it is achieved by lipostructure, also called lipofilling or autologous fat transfer: fat is harvested from the abdomen or thighs, purified and reinjected around the shaft.
The diameter can also be increased with hyaluronic acid injections: this is non-surgical penoplasty, performed in the consulting room without general anaesthesia.
Lengthening penoplasty, or ligamentolysis, is only effective on the flaccid penis. Hyaluronic acid injections increase the length and, above all, the girth of the penis, directly in the consulting room under anaesthetic cream. The gain is immediate, the session lasts about 45 minutes and recovery is simple, with no downtime and no sick leave. The result lasts 18 months on average before an optional touch-up to prolong the effect.
Surgical penoplasty combines an autologous fat transfer around the corpora cavernosa (lipofilling) and, when needed, the release of the suspensory ligament. Performed under general anaesthesia as an outpatient procedure, it offers a permanent result on girth and on flaccid length, with a simple recovery over a few weeks.

Surgical penoplasty is performed in the operating theatre as an outpatient procedure: you go home the same day after a few hours of monitoring. For girth enhancement, fat is harvested with a fine cannula through well-hidden incisions on the abdomen or thighs, then purified by washing and centrifugation (Coleman technique) or filtered (Puregraft technique). It is reinjected through tiny incisions, as microparticles, between the skin of the shaft and the corpora cavernosa. Lipostructure is a permanent method. The operation lasts between one hour and one hour and a half.
For lengthening, a discreet incision is made at the base of the penis (Z-plasty or V-Y plasty). Through this incision the surgeon releases the suspensory ligament of the penis.

Penoplasty is a well-codified procedure with a favourable safety profile when performed by an experienced surgeon. As with any medical act, complications are possible. Penoplasty does not affect erectile function, fertility or orgasmic sensitivity when performed according to good practice. No functional anatomical structure (corpora cavernosa, urethra, dorsal nerves) is cut during these procedures. Sport, cycling, motorcycling and horse riding are not recommended for 4 to 6 weeks to avoid any trauma to the operated area.

The corpora cavernosa, the urethra and the dorsal nerves of the penis are not cut. In some cases, the thickening of the tissue linked to girth enhancement can even reduce premature ejaculation.
A slight, usually temporary, change in sensation is possible because of the thickening of the tissue around the shaft. Sensitivity generally returns to normal within a few weeks. In men with premature ejaculation, this slight decrease in sensitivity can be an advantage.
Fat transfer uses your own fat, harvested by liposuction from the abdomen or thighs, so the result is permanent and 100% natural, without any foreign product. Hyaluronic acid is resorbable, which is an advantage if you want to test the effect before committing to something permanent, but it requires regular touch-ups.
