Lower blepharoplasty is a cosmetic surgery procedure performed on both women and men that is commonly performed in the 40s. However, it is sometimes performed much earlier, when the disfigurements are constitutional (hereditary factors) and not age-related, such as certain “fatty bags”.
We perform the treatment of bags under the eyes without scarring.
For patients who do not wish to undergo surgery, it is possible to treat bags under the eyes with hyaluronic acid injections.
This information sheet was written in June 2021 by Dr Ralph ABBOU who is a plastic surgeon in Paris.
Lower eyelid blepharoplasty refers to cosmetic surgery of the eyelids that aims to remove bags under the eyes, whether they are hereditary or due to age. They can concern only the two lower eyelids, or be performed at the same time as an upper blepharoplasty.
A lower blepharoplasty can reduce the signs of aging in the lower eyelids and replace the “tired” look of the eyes with a more rested and relaxed appearance.
To all patients who complain about :
Two medical consultations with your plastic surgeon to plan your operation. An ophthalmological assessment may be prescribed.
The surgical procedure takes place in the operating room under general anesthesia and lasts an average of one hour.
Each surgeon adopts his own technique, which he adapts to each case to obtain the best results. The following basic principles can be retained:
First of all, the skin incisions are placed 1 to 2 mm under the eyelashes, and can be extended a little outside. In the case of isolated bags under the eyes (without excess skin to be removed), a blepharoplasty can be performed through the trans-conjunctival route, thus leaving no scar on the skin.
Then the fat pockets are removed after coagulation.
Finally, a small spindle of skin can be removed through the subciliary incision
There is no real pain, but a certain discomfort with a feeling of tension in the eyelids, a slight irritation of the eyes or some visual problems. The post-operative period is essentially marked by the appearance of edema (swelling), ecchymosis (bruising) and watery eyes, the extent and duration of which vary greatly from one individual to another. The return to normal life is possible after a few days (6 to 20 days depending on the case). The scars may remain slightly pink during the first few weeks, but their make-up is quickly authorized from the 7th day.
A delay of 3 to 6 months is necessary to really appreciate the result. The operation will most often have corrected the sagging skin and eliminated the bags under the eyes, thus rectifying the aged and tired appearance of the eyes. The results of a blepharoplasty are generally among the most durable in cosmetic surgery.

The fat is removed by laser from the inside of the eyelid without any visible scarring. Upper and lower eyelid surgery can be performed together at the same time.
Dark circles can be treated with lower eyelid surgery if they are the result of puffiness due to swelling of the fat or bags. If the dark circles are the result of pigment changes in the eyelid skin, chemical peels can improve the coloring.
Most of the time, there is no visible scar after healing. The incisions in the eyelids are made in the natural folds of the skin. Once healed, the scars fade, leaving a very discreet incision line that becomes almost imperceptible after healing.
Generally speaking, the benefits of a blepharoplasty are long-lasting and few people need to repeat the procedure. The fat that has been removed will not return. Of course, as we age, the skin becomes looser and may hang down. The procedure cannot alter or delay the natural aging process. But the benefits of the procedure last 15 to 20 years on average.
