In the closed rhinoplasty approach, also called endonasal rhinoplasty, the surgeon positions the scars inside the nostrils. Although an external scar is avoided, considerable challenges and technical limitations are associated with the relative lack of surgical access.
Unlike closed rhinoplasty, the open rhinoplasty approach uses a small incision, called a transcolumellar incision or rethi route to connect the right and left nostril incisions. This allows the nasal skin to be folded upward and full visibility of the lower nasal skeleton is achieved. Thus, the hallmark of open rhinoplasty is the greatly improved surgical access afforded by the transcolumellar incision.

It is important to keep in mind that the final result will only be obtained after six months to one year.
