Septoplasty: correcting the deviation of the nasal septum
Some patients are looking for an improvement in breathing without changing the nasal contour or shape of their nose, hence the term functional rhinoplasty or septoplasty.
The most common case in rhinoplasty is the patient seeking both functional and aesthetic improvement.
After failure of the medical treatment carried out most often by an ENT (ear, nose and throat specialist) to improve the permeability of the nasal cavities, a surgical intervention can be considered: septoplasty.
This information sheet was written in June 2020 by Dr Ralph ABBOU who is a plastic surgeon in Paris.
Dr. Ralph ABBOU defended his medical thesis on the subject of rhinoplasty and has published several scientific papers on this subject.

What is septoplasty ?
Septoplasty is a surgical procedure that repairs a deviated septum, i.e. a deviation of the nasal septum.
The septum is the wall of bone and cartilage that divides the inner part of your nose into two separate nostrils. Some people are born with a deviated septum, or have a deviated septum after a nose injury or trauma. Most people with this condition have a much smaller nasal airflow on one side than the other. This can cause difficulty breathing.
Deviation of the nasal septum can result in:
- nasal obstruction
- snoring or promote a sinus infection and sometimes ear infection.
What are the objectives of septoplasty ?
Septoplasty allows the reshaping of the nasal septum by correcting the deviation, to improve breathing. The result obtained will depend on the importance of the anatomical deformation observed. It corrects the deviation of the nose in order to balance the flow of incoming and outgoing air.
Who are the candidates for septoplasty?
Septoplasty is intended for patients with symptoms of functional airway obstruction:
- nasal congestion or blocked nose: the patient complains of difficulty breathing through the nose
- mouth breathing (especially at night)
- dry mouth
- limited sense of smell.
How is the treatment of deviated septum performed?
Before the intervention
- Two medical consultations with your surgeon to examine you and plan your surgery.
- In some cases, you may be asked to undergo an ENT examination and a CT scan of the facial mass.
- Anesthesia consultation and preoperative blood work
Procedure of the septoplasty
The septoplasty procedure is performed under general anesthesia through the natural channels, without any skin scar, except in exceptional cases where it may require a minimal, hardly visible skin incision.
Here are the different steps of the septoplasty:
- Straightening of a deviated septum
In nasal septoplasty, the dividing wall separating the right and left nasal passages is curved, deviated, arched or twisted. Septoplasty uses various surgical techniques to straighten the septum.
- Turbinoplasty for horn reduction
Enlarged turbinates are another common cause of nasal obstruction, especially in patients with long-standing nasal allergies. Turbinoplasty is also a simple surgical procedure that can significantly improve nasal airflow. Recovery is usually quick and painless.
- Removal of nasal polyps
Nasal polyps are benign grape-like growths that sometimes block the nasal passages. In benign cases, removal is simple, but the likelihood of recurrence is often high.
Although removal of internal airway blockages is often sufficient for some patients, more severe forms of nasal airway obstruction require surgical treatment of the external nasal structure. In this patient population, rhinoplasty (cartilage grafting) techniques are necessary to straighten, enlarge and strengthen the blocked external nasal passages to create an unobstructed airway between the nostril and throat.
Post-operative care
The aftermath of septoplasty is not very painful. Bruising and swelling of the face may persist for a few days.
The improvement of the nasal flow is perceptible from the 15th postoperative day onwards on average.

Can septoplasty treat snoring?
In some cases, septoplasty (correction of a deviated septum) can reduce or stop snoring. In most cases, septoplasty does not treat snoring. A specialized ENT evaluation is preferable if you wish to treat snoring effectively.
How to sleep after a septoplasty?
All bits in the nose will be removed before you go home. You should sleep with your head elevated to help reduce postoperative swelling.
