What are breast implants ?

Breast prostheses are implants or medical devices used to increase breast size in cosmetic and plastic surgery.

Silicone or saline filled implants can add volume to your breasts, improving their size and shape.

 Breast implants are among the most studied medical devices. Their technology has also improved considerably in recent years, with options that look more natural and carry fewer risks.

Here’s how the breast implant procedure works

Before breast implants:

Process of an operation

The procedure takes place in the operating room under general anesthesia and lasts about an hour. The patient is discharged the same day or the next day, depending on the case.

When you wake up after the procedure, there will be some discomfort and moderate chest pain. In addition, your movement will be limited. This is especially true if the implants are placed under the muscle. However, most patients find this discomfort quite tolerable and will disappear after a few days.

For a breast augmentation by prostheses, there is necessarily a scar that is placed in an inconspicuous area.

If your breasts have moderate ptosis, then breast augmentation with prostheses can improve their shape and form. If the ptosis of the breasts is significant, a breast lift will be the most appropriate surgical solution.

There’s still a lot that plastic surgeons don’t understand about capsular contracture, so there’s always a chance that you could develop it. However, there are steps you can take to reduce your risk :

  • The “no contact” technique when inserting your breast implant. 
  • Place your implants in a sub mammary incision.
  • Stop smoking before the procedure.

Silicone gel-filled breast implants are the most tested medical device in the world. Their safety has been proven in numerous studies over the years.

There is actually a scientific method for selecting your breast implant size based on the width (base) of your breasts and the projection you desire.  Complete breast measurements are imperative to ensure that you receive the appropriate size. Of course, you can select an implant that is larger or smaller than your breast measurements, but to a certain extent, otherwise the result will not be optimal. 

A round breast prosthesis provides more volume on the upper part of the breast and allows a more rounded result than average for a more pronounced neckline.

An anatomical breast prosthesis provides more volume on the lower part of the breast, which allows for a more natural result.

These explanations are theoretical to allow you to understand the differences between the two types of breast implants. Depending on your desire and your morphology, we will find the prosthesis that will suit you.

Because modern scientific research proves the safety of silicone breast implants, most women choose silicone implants over saline-filled breast forms. However, each type of implant has its advantages and disadvantages. Silicone serum implants tend to lose some volume every year. In practice, 99% of the implants placed are silicone gel implants. We will advise you in consultation according to your case.

Textured breast implants theoretically have a lower incidence of capsular contracture or shell.

When implants are placed above the pectoral muscle, many breast surgeons choose textured breast implants. When implants are placed below the pectoralis muscle, many surgeons choose a smooth breast implant.  

In practice, for the past year or so, we have been placing only smooth implants because of the risk of anaplastic large cell lymphoma that has been shown to occur with textured implants. To ensure your safety, we follow the official recommendations of the French National Authority for Health.

  • Underarm: Using modern endoscopic equipment, a new site for inserting breast implants is through the underarm. The scar is well hidden in the natural folds of the armpits. The advantage of the underarm approach is the absence of scarring on the breasts.

Silicone gel gives a natural feel close to a natural breast and in case of rupture, it is cohesive, meaning that the gel does not disperse in the body: it is the reference implant today.

Saline breast implants have a shorter life span (10 years maximum) and present a greater risk of leakage than silicone gel implants. These implants are rarely used today.

Breast implants above or below the pectoralis major muscle ?

Numerous research studies have shown that a breast implant placed under the pectoral muscle has a lower risk of becoming hard or developing capsular contracture (shell) than an implant placed over the muscle. The extra layer of tissue or muscle covering the breast implant will further camouflage any rippling of the breast implants.

A potential problem with placing a breast implant under the muscle is called animation deformity, in which the shape of the breast implant is distorted each time you squeeze the pectoral. If you have sagging breasts, a breast implant placed over the muscle is better able to lift the breasts than one placed under the muscle.

Breast implants can be positioned either :

  • In front of the pectoralis major muscle
  • Behind the pectoralis major muscle

The choice of the position of the breast implants depends on many parameters that will be explained to you during the medical consultation and that depend on your wishes and your morphology.

To simplify:

  • If you already have breasts, it is possible to place the implants above the muscle and this will give a very natural and harmonious result.
  • If your chest is really small, we advise you to place the prostheses behind the muscle, and this will allow a more natural result in this case. Indeed, if we place the implants above the muscle in a patient who has no breast, the implants may be very visible.
