The treatment of baldness in cosmetic surgery

The treatment of male and female baldness is a specialized area of facial plastic surgery that includes several types of procedures: hair transplantation, forehead reduction, scalp mesotherapy, female baldness treatment and beard transplantation.

Androgenic alopecia is the medical term for “young male pattern baldness”.  This diffuse baldness is often attributed to a genetic predisposition to hereditary baldness:

  • Early baldness (from 18 years of age).
  • Extensive and excessive alopecia.
  • The evolution can be a complete baldness.

Female pattern baldness: we now know that there is androgenetic hair loss in women that begins around the age of 40 and affects the top of the head. This female baldness requires special care because of the particular hair cycle in women.

This information sheet was written by Dr. Ralph ABBOU and Dr. PESSIS who are two cosmetic surgeons in Paris who regularly perform baldness treatments for men and women using surgical and non-surgical techniques.

Before treatment

  • 2 Cosmetic surgery medical consultations to analyze your hair loss, understand your needs, and define a medical and surgical strategy adapted to your case: Stage of baldness, hair in anagen or telogen phase, condition of hair follicles, lifestyle, possible causes of baldness.
  • Anesthesia consultation if general anesthesia
  • Stop smoking
  • Stopping anti-coagulant and anti-inflammatory treatments

Course of treatment

Depending on the stage of your baldness and your request, I will propose a surgical strategy based on the following techniques

Before treatment

  • FUE or FUT (strip) hair transplants to fill in the bald areas of the head.
  • Mesotherapy of the scalp to increase the density and thickness of hair

 The aftermath of baldness treatment

The aftermath of the hair transplant is not painful.

You can resume your activities about a week after the hair transplant procedure.

Hair growth is a slow process, even in a person who has no hair loss. After the procedure, the first signs of growth of the implanted hair may take three to four months. About a year after the operation you will see the final result

The transplanted hair follicles are genetically very resistant, so they will continue to grow throughout your life.

It is possible to perform several hair transplant sessions with a rest period of 3 months between each session.

Mesotherapy of the scalp allows an immediate return to your activities, there is no convalescence.

FAQs

What are the medical treatments for baldness?

These treatments are effective at an early stage. In more severe cases, surgical treatment of hair loss is often the solution.

  • Propecia: This treatment comes in tablet form and is part of a class of drugs called 5-alpha-reductase inhibitors. Le propecia permet de bloquer la perte des cheveux.
  • Minoxidil: is a topical treatment in the form of a lotion that is applied directly to areas of hair loss. Minoxidil stimulates keratinocyte growth and hair growth in patients with alopecia androgénétique.
  • Medical LED: LLLT (low level laser therapy) consists of medical light therapy sessions performed in the doctor’s office to stimulate hair growth and reduce follicle inflammation.

What medical treatment is effective for hair regrowth?

There is no miracle cure for hair loss. There are only 2 molecules tested and proven today in the treatment of alopecia, Minoxidil and Finasteride. These molecules are effective at an early stage but when baldness is important, surgical treatment is the reference solution.

How to fight against the beginning of baldness?

The gold standard of treatment for early baldness is scalp mesotherapy, which can slow down hair loss and stimulate hair growth.

Most male and female baldness can only be prevented through medical or surgical treatment, but there are ways to be gentler with your scalp that will help preserve your existing hair. “Baldness is a genetic condition, but it can be exacerbated by excessive stress, poor circulation, poor diet, pregnancy, drug use, illness, thyroid problems… Some of these are preventable and some are not, so following a healthy lifestyle and managing stress can only help your hair. Here are some other key ways to reduce hair loss:

  • Avoid adding stress or trauma to your scalp or follicles with hairstyles such as braids or ponytails or by pulling your hair directly
  • Be gentle when washing, brushing and styling your hair.
  • Use a wide-toothed comb that won’t pull too hard.
  • Avoid overheating your hair or scalp with styling tools and products.
  • If you can, avoid medications and supplements known to cause hair loss.
  • Don’t wear tight hats or headbands.
  • Quit smoking. Studies show a correlation between smoking and hair loss

How does baldness start?

Baldness is mainly a male phenomenon. Hair loss appears on average between the ages of 20 and 35, but can begin later. The warning signs are abundant hair loss, with a progressive clearing of the forehead, the top of the temples or the top of the skull.

What is PRP (platelet-rich plasma) used for?

PRP (platelet-rich plasma) for hair loss involves taking a small amount of your own blood, processing it in a centrifuge to isolate the platelets and injecting it into your scalp. The growth factors in the platelets are thought to increase blood flow to the hair follicles and lengthen the anagen (active) phase of hair growth. Research has shown that PRP is effective in treating both age-related hair loss and alopecia areata. Most patients require three to four treatments, one month apart, to see a benefit. We recommend follow-up sessions once or twice a year to maintain your results.

What is Neograft?

NeoGraft is an automated device approved for FUE hair transplant surgery. This portable device harvests individual hair follicles for implantation in bald areas. With NeoGraft, an FUE transplant can take four to six hours – possibly longer, divided into two sessions on different days if you have very severe baldness. You can expect to need two to three days of recovery time before returning to work.

What is the purpose of the Artas robot for hair transplantation?

The ARTAS system is a robotic version of the FUE hair transplant technique. The system selects groups of follicles with the largest number of hairs and harvests them in a way that preserves the appearance of the donor site. The follicles are then meticulously placed, robotically or manually, in thin or bald areas. “The advantage of the ARTAS system is that it is less stressful for the surgeon than manual follicular unit extraction techniques” The robot produces “more consistent results compared to individual surgeons using manual techniques with varying levels of skill.” However, ARTAS needs the supervision of an experienced hair surgeon in order to create good results. With any FUE procedure, new hair begins to grow back in about three months, and results continue to improve over the first year.

Can baldness be treated?

There is no miracle treatment for baldness. In cosmetic surgery, the symptoms can be treated by grafting hair on bald areas. The results of baldness treatments last for several years.

How to stop the onset of baldness?

Mesotherapy of the scalp or capillary mesotherapy can slow down or stop hair loss.

How do you know if you have a bald spot?

We speak of baldness when hair loss is visible. Baldness generally begins in the area of the gulfs (forehead) and in the area of the tonsure.
